A Case of Functioning Adrenal Rest Tumor of Liver

Abstract
drenal rest tumor is an aberrantly located collection of adrenocotical tissue which has
been most commonly described in abdominal and pelvic sites. Occasionally adrenal rest
tumors have been reported to occur in the liver. We report a case of a large hypervasular
hepatic tumor in 32-year-old female patient presented with virilization and inferility. Serum
testosterone and dehydroepiandrosterone sulfate (DHEA-S) levels were elevated and there was
an increase in urinary 17-ketosteroid and free cortisol. Abdominal CT scan showed a huge
mass in posterior segment of right lobe of liver. Celiac angiography and abdominal
aortography showed a hypervascular tumor in right lobe of liver, which was mainly
supplied by right hepatic artery and parasitically supplied by right adrenal and inferior phrenic
arteries. Venous drainage of tumor was viaright hepatic vein. But selective venous
sampling by inferior vena cava catheterization failed to localize the site of hypersecretion
of steroid hormone levles. In operative findings, both adrenal glands were intact. Tumor was
found in posterior surface of right lobe of liver. After right lobectomy of liver, the serum
DHEA-S level was decreased to normal range by post-op 4th day. To our knowledge, this is
the first case of an adrenal rest tumor of the liver proved to be functionally active in Korea.
